To initiate event with Tuna you need to have Toilet Gangbang passed before and be at the Vault at late evening or later and speak to Tuna (you will see new dialogue option). Might need to pass a day to get this working.
New event with Kelly is initiated via 'Kelly Bored' event drinking at Vault. Ask her about her boyfriend and Casey will get an option about purple dildo memories. This will lead eventually to going back to the office, where you will be able to initiate girl on girl or girl on girl + Sam. Need to have Kelly and Casey #1 passed before that.
